If your child has an upcoming first time consult appointment related to behavior, please fill out the Vanderbilt form for parents and also have your child’s teacher(s) fill out the Vanderbilt form for teachers. Once complete, please submit these forms to our office. Upon receipt, your doctor will review and score them at which point we will contact you to schedule your behavior consult. If you haven’t heard back within 7 days of submitting your forms, please contact us for a status update.
